<\/p>

Registration<\/b>Upon arrival, we kindly request that you register at the reception desk. According to the Healthcare Insurance Act, you must identify yourself with a valid form of identification when receiving medical care. Therefore, please bring a valid passport, identity card, or driver’s license to your first appointment. <\/p>

Your personal data<\/b>At your first appointment, we ask you to bring a valid proof of identity that includes your BSN (Citizen Service Number). This can be a valid passport, European identity card, or driver’s license. Asylum seekers must bring their RZA card, passport, and foreign national document. <\/p>

Changes to medical data<\/b>To ensure your safety and proper treatment, we request that you report any medical details that have not been previously discussed during each visit. This includes pregnancy, medication use, illnesses, allergies, and other physical or mental conditions. The practitioner will inquire about this at every appointment. <\/p>

Privacy policy and confidentiality of patient data (GDPR)<\/b>Your personal information is treated with strict confidentiality and remains within Tandartspraktijk We Smile, unless otherwise agreed with the dentist or if necessary for medical research by a specialist or general practitioner.<\/p>

Be on time<\/b>To keep the schedule as efficient as possible, we request that you arrive on time for your appointment. Occasionally, a treatment may run slightly over, causing your appointment to start later than planned. We ask for your understanding in such cases. <\/p>

Periodic check-up<\/b>For optimal oral care, we advise patients to come for a check-up 1 to 2 times per year. If you have not had a check-up for more than 2 years, you will be automatically deregistered. You must then first schedule an intake appointment to record the current status of your mouth and establish a treatment plan. <\/p>

Making an appointment<\/b>Making an appointment is simple. You can schedule an appointment at the desk immediately after your check-up. If this is not possible, you can contact us by telephone during our opening hours to make an appointment. <\/p>

Email and SMS service<\/b>After making an appointment, you will receive a confirmation by email. A few days before your appointment, you will also receive a reminder email. Additionally, we will send you an SMS message as a reminder of your appointment. If for any reason you do not receive an email or SMS, the appointment remains scheduled for the agreed date and time. <\/p>

Canceling or rescheduling an appointment<\/b>If you wish to cancel or reschedule your appointment, we request that you notify us at least 24 hours in advance. We prefer to receive cancellations by telephone. For late cancellations or failure to appear at the agreed time, we may charge a fee. <\/p>

Emergencies<\/b>In case of dental pain, we strive to help you as quickly as possible. During our working hours, you can call us or drop by. Outside of our working hours, we refer you to the Tandartsspoedpraktijk Spoedpoli Leiden at 0900 8602. <\/p>

Staying at the practice<\/b>Upon arrival, we request that you report to the desk so that your presence is noted. It is possible that an appointment may take longer than planned. If you arrive late for your appointment, we may have to reschedule it due to lack of time. We may charge for the appointment costs. <\/p>

Watch over your belongings<\/b>We advise you not to leave your belongings unattended. Tandartspraktijk We Smile is not responsible for damage, theft, or loss of your property. Aggression and violence are not tolerated and may lead to police involvement and deregistration from the dental practice. <\/p>

Your practitioner<\/b>Various dentists, dental hygienists, and prevention assistants work at Tandartspraktijk We Smile. You may indicate your preference for a specific practitioner at the reception; where possible, we will take this into account. <\/p>

Estimate<\/b>For treatments where the costs are expected to exceed \u20ac250, you will receive an estimate. This estimate is a preliminary calculation and may differ from the final costs. This can be due to adjustments in the treatment during its execution, among other reasons. <\/p>

Customer satisfaction<\/b>Your feedback is valuable to us. You can leave your review on our website or pass it on to a specialist or desk staff member. We continuously strive to improve our services. <\/p>
